AIM Academy partners with Magnus Health SMR to streamline management of required student forms including health-related forms and support services contracts using a secure, HIPAA-protected system. To complete necessary forms, please click here to access the Magnus Health Portal (your username and password are the same as your Parent Portal login) or follow the link found in the Parent Portal. Students will not be permitted to attend classes or participate in any school activity, including athletics, until all medical, dental and electronic consents are completed.

For planning purposes updated dental forms are required for all 1st, 3rd and 7th grade students and updated medical forms are required for all students entering 1st and 3rd grade as well as 6th through 12th grade. For new students entering 2nd, 4th and 5th grade, health records from your child’s former school can be uploaded into the system.
